Blog Layout Order Form

I've been having a hard time managing the specs of some requested layouts, so I'll be asking anybody who wants a custom layout to please fill up this form on NOTEPAD and send to adahikari@yahoo.com. For extra requests/specs/consultation that you want, it'd be best if you add me on YM, adahikari so we can talk directly.

Basic Ordering Form for Custom Layouts:

Other: please specify (price depends on effort)

Sections on sidebar: (Specify name of each section and if you want a special graphic header for each.)
Generic Image Headers (50, >7; 75 <7)
Specialized Image Header per section (10 per header)

I will be adding sample works here on my site as soon as the perios are over, most likely during sembreak. Please be patient with your orders as most of the time, they pile up and I can't do anything about it.
